You must make sure that we can clearly read all information on the game report, especially the date, game #, etc. If game reports have not been properly signed or if we do not receive these reports on time then you will not be compensated for the mid-season paycheck. We will only accept the new game report form. Do not send in other reports as these will not be used or considered for compensation.
